Supporting People Floating Support Form (PDF)

Which form do I need to fill in?

Rough Sleeper

If your client is Rough Sleeping then please fill in the ‘Rough Sleeper and Street Population Form’.

As a rule if your client was rough sleeping last night and will be rough sleeping tonight this will be the correct form to fill in. The contact information and how to send the completed form back to Porchlight are on the bottom of the form itself.

Outreach

If your client is sofa surfing, in unsuitable accommodation or threatened with homelessness then the ‘Supporting People Floating Support Form’ will be the most appropriate form to fill in.

It is important that you mark clearly that your client is, for example, sofa surfing, on the front of the form so that Supporting People can allocate the individual to a suitable provider.

Please be aware that the Supporting People forms should be sent direct to Supporting People using the address and contact information provided on the form.

Porchlight can still take referrals and give advice over the phone for either service and will forward any referrals for the outreach service to Supporting People in this case. The Freephone number is 0800 567 76 99.
