Hours: 18.75 hours per week (some flexibility required)
Salary: £10,139 - £11,448.50 (£20,278 - £22,897 full time equivalent) gross per annum

The Primary Care Community Link Service is part of Porchlights Live Well Kent Services. The service works alongside GP Practices as well as other agencies across the county to support people who are experiencing social isolation and exclusion. We also assist with any social issue that may be impacting upon the Individual’s mental wellbeing.
The service provides individually tailored, one to one, and time limited support for people with mental health needs. As a link worker, you can expect to be supporting people to engage in suitable activities across a number of areas including mental and physical health, education/training, housing and money, building confidence and managing relationships. The service is underpinned by the philosophies of recovery and social inclusion, with the successful candidate being required to uphold the values and ethos of the organisation in their work.
The successful candidate will have excellent communication and interpersonal skills, the ability to develop effective working relationships with colleagues, clients and internal and external agencies and the ability to handle conflicting demands. Interviews are due to be held 18th January in Maidstone.
You should have experience of supporting people with mental health needs in the community and working with a range of statutory and voluntary agencies.
