Primary Care Community Link Service
Our Primary Care Community Link Service helps people manage any issues that are affecting their day to day life or their ability to live independently. We do this by connecting people to support in their local area.
We provide one to one advice and guidance that is completely free for anyone aged 17 and over.
We can help you with
- Your mental & physical health
- Finding things to do
- Your own space
- Education, training & volunteering
- Drugs and alcohol
- Your relationships
Whatever you’re facing, we’re here to help. We won’t make judgements and what we discuss will remain confidential.
Without the support of my link worker, I would have ended up homeless. I had nowhere to turn and no idea how to get the help I needed, but she helped me find the right support so that I could look after myself properly.
Where can I get support?
Your link worker will meet you at your local GP surgery or somewhere within the community to talk about the difficulties you are facing. We’ll work with you to set some goals and actions to help you move forward. You choose what we help you with and what activities you would like to access locally.
